Home Based Palliative Care
Our Home Based Care Team travels daily to client’s homesteads, serving both patients and families. The holistic services they provide at home are based on an assessment of the physical, psychological, social, and spiritual needs of the patient and his or her family. We then provide care according to the needs of the patients. We also work with local medical clinics to provide the patient with relief from pain and control distressing symptoms. Patients needing more intensive care are referred to the Hospice.
Duduza’s HomeBased Care programme is managed by a professional nurse, an enrolled nurse, a senior counsellor, and 25 caregivers and HCT counsellors.
Working with clients in their homes provides opportunities for our staff to promote wellness, by providing health information regarding a variety of health topics including TB, HIV/AIDS and other chronic illnesses like hypertension (high blood pressure), stroke, diabetes and more. Treatment adherence is emphasized, as is nutrition education and strategies to prevent health issues like diarrhea.
